Become A New Patient

Our goal is to love and serve every patient that walks in the door.

We offer a part-time free primary care, diagnostic testing, disease management, pharmacy, and case management assistance for those who are uninsured or underinsured. Our clinic physician appointments are on Thursday evenings. Case management and diabetic coaching are available on other days and times.
Call 405-759-0853 (select 1) for an appointment or more information.  If you would like to fill them out prior to your appointment, you may download and print the new patient form by clicking the button below . Please bring a list of your current medications.

*Patient Policies:

  • We do not stock or distribute any narcotics through our pharmacy.

  • Prescription refills must be called in by 5 PM on Sunday prior to Thursday evening for your refill(s) to be available. Our volunteer pharmacists refill hundreds of prescriptions prior to Thursday evening clinic. The Pharmacy opens at 6 PM on Thursday evening with a curbside service. Our Pharmacy Techs will come to your vehicle. Chaplains are also available for curbside ministry.

  • Cancellations must be made 24 hours prior to the scheduled appointment so we can schedule another patient who needs assistance. Patients who do not show up for their appointment are subject to a suspension from all services (including prescriptions). A “No show” prevents us from providing healthcare for other patients who are on our wait list.

  • We do not discriminate on the basis of anything. We believe in the sanctity and dignity of all human life and desire to share God’s love in a tangible way.

  • We reserve the right to dismiss any patient who becomes verbally or physically abusive or threatening toward our volunteers.

*PATIENT NO SHOW POLICY UPDATE:

  • We understand that our patients may need to cancel an appointment occasionally. In such circumstances, please contact us no later than 24 hours before your scheduled appointment time. You may do so by calling 405-759-0853.

  • If you do not show up for your appointment, or cancel or reschedule within 24 hours of your appointment time, we will consider that a no-show.

  • Two No-show appointments will result in removal as a patient. You may reapply after 12 months.

  • We know that unexpected situations sometimes arise. We have a waiting list and want to help us many of our neighbors as possible. Your No-show wastes the valuable time and resources of our volunteers and prevents us from helping another patient in need of medical care. Please consider others if you are unable to make your appointment.
